Counseling Services & Informed Consent

Healthy Growth Mindset, LLC is committed to providing ethical, respectful, and collaborative mental health services. Counseling is a voluntary process in which clients are encouraged to participate actively in treatment planning, ask questions, and make informed decisions about their care. Clients are encouraged to discuss any questions or concerns regarding counseling services with their clinician.

Privacy & Confidentiality

Healthy Growth Mindset, LLC maintains the privacy and confidentiality of client information in accordance with applicable federal and Texas laws, including HIPAA. Detailed information regarding the collection, use, disclosure, and protection of personal and health information is provided in Section 2: Privacy Practices & Texas Consumer Notice.

Client Rights & Responsibilities

Clients are entitled to respectful, ethical, and professional care and are encouraged to participate actively in the counseling process. A complete description of client rights and responsibilities is provided in Section 3: Client Rights & Responsibilities.

Client Grievance Procedure

Clients may express concerns or submit grievances regarding services without fear of retaliation. Written grievances may be submitted to:

Emergency & Crisis Policy

Healthy Growth Mindset, LLC does not provide 24-hour emergency or crisis services.

If you are experiencing an immediate emergency, call 911 or seek assistance at the nearest emergency department.
For mental health crisis support, call or text 988, the Suicide & Crisis Lifeline.

Telehealth Policy

Telehealth services involve the use of electronic communication technologies to provide counseling remotely. Clients should understand that telehealth may involve technological limitations, interruptions, and potential privacy or security risks. Clients must be physically located in a state or jurisdiction where their clinician is legally authorized to provide services. Telehealth is not a substitute for emergency or crisis services. The emergency procedures described above remain applicable.

Fees, Billing & Cancellations

Clients will be informed of applicable service fees, billing practices, payment expectations, and cancellation policies before receiving services.Clients are encouraged to ask questions regarding financial responsibilities and scheduling requirements.

Communication & Social Media

Healthy Growth Mindset, LLC maintains professional boundaries in all client communications. Social media platforms and public websites are not appropriate channels for clinical communication. Clients should use practice-approved communication methods for matters related to treatment.

Accessibility Statement

Healthy Growth Mindset, LLC is committed to providing accessible and inclusive mental health services. Reasonable accommodations are available upon request, consistent with applicable law. Clients are encouraged to communicate accessibility needs so that appropriate arrangements can be considered.

Client Responsibilities

Clients are responsible for:

• Providing accurate and complete information relevant to treatment.

• Participating honestly and respectfully in the counseling process.

• Attending scheduled appointments and providing required notice for cancellations.

• Communicating openly about concerns, goals, or changes in circumstances.

• Respecting professional boundaries and established communication policies.

• Using appropriate emergency services (911 or 988) during crisis situations rather than routine clinical communication.

• Treating clinicians, staff, and others with courtesy and respect.
